CBP Form 5106 Guide: Creating or Updating an Importer Identity

The Create/Update Importer Identity Form (CBP Form 5106) is used to create or update importers' unique identification information in CBP's systems.

Please note that this form is only necessary for customers shipping to the United States. 


1. Header & Identification

  • TYPE OF ACTION: ✔Notification of Identification Number

  • 1 A. Importer / Business Name:  your name or company name

  • 1 B. Identification Number

    • Tick the SSN box (leave “Request CBP-Assigned #” blank unless you prefer to mask your SSN).

2. Address & Contact

  • 2 A. Mailing Address

    • Street, City, State

    • Type of address: ✔ Residential

  • 2 C. Phone:

  • 2 E. E-mail: 

3. Additional Data is required when the importer is a corporation

*If you will use your SSN as the Importer ID for personal (non-commercial) imports, you can generally leave all items in Section 3 “Company Information” (Blocks 3A–3J) blank.

  • 3 A. Business Description

  • 3 B. NAICS Code

  • 3 E. Year Established

  • 3 J. Contact Person / Officer

4. Certification

  • Printed or Typed: Full Name and Title
  • Signature
  • Date
